Request for an Authorized W-9 form from the University of New Mexico

Form W-9 Request for Taxpayer Identification Number (TIN) and Certification is the standard form published by the Internal Revenue Service (IRS) used by payers to request TIN information. A person, company, or agency making payment to UNM, and who is required to file an information return with the IRS, must obtain the University's TIN in order to do so.   Although UNM is exempt from information return reporting, UNM will still get requests for form W-9, and will provide this information to the requestor. In general, requests for UNM's Taxpayer Identification Number are made by payers in order for them to properly report information in connection with miscellaneous payments made to the University (class tuition, services, interest, dividends, rents, royalties, etc.)  

UNM’s Employer Identification Number (EIN) is 85-6000642. 

However, any donors who are requesting a W-9 to enable  them to claim tax deductions for gifts and donations must request a W-9 from the UNM Foundation, which has a different EIN than UNM.
 

When a department receives a request for the University's Taxpayer Identification Number (TIN), for sales of services or goods to an outside entity, the process for responding on behalf of the University is as follows:

UNM Departments can obtain an authorized UNM W-9 form "Here."  A UNM NetID and Password is required.

If any additional information is being requested on the form, such as remittance address, contact information, or substitute W-9, the department should fill out the new form and sign it; but it will only be valid if the authorized UNM W-9 form provided at this link is attached.

Outside Entities requesting a UNM W-9 should contact the UNM Department that they are dealing with directly. Your substitute W-9 or Vendor form should be accompanied by the authorized federal UNM W-9 form.
